The Immigration Health Surcharge (IHS) is a fee imposed on non-UK residents applying for a visa to enter or remain in the United Kingdom for more than six months. It grants a person access to NHS services on par with UK residents during their stay.
Who is required to pay the IHS?
Most visa applicants intending to stay in the UK for over six months are required to pay the IHS. This includes individuals applying for work, study, or family visas. However, certain groups are exempt, such as:
- Applicants for a Health and Care Worker visa.
- Dependants of members of the UK armed forces.
- Family members of EU nationals with settled or pre-settled status under the EU Settlement Scheme.
- Diplomats and their dependants.

Do I need to pay the IHS if I am visiting the UK?
Fortunately, you do not need to pay the surcharge if you are applying for a visitor visa. Persons applying for visas for six months or less from outside the UK are also exempt.
Do I need to pay the IHS if I am applying for Indefinite Leave to Remain or British Citizenship?
These applications are exempt, and applicants do not need to pay the IHS. However, if you apply for Indefinite Leave to Remain and are only given limited leave, you will have to pay the IHS before you are given the leave.

How much does the IHS cost?
The standard IHS rate is £1,035 per year for most applicants. However, discounted rates apply to certain groups:
- Students and Youth Mobility Scheme applicants: £776 per year.
- Children under 18: £776 per year.
The total IHS fee is calculated based on the total duration of the visa. For example, a three-year work visa would incur an IHS fee of £3,105 (£1,035 x 3 years).
When and how is the IHS paid?
The IHS is paid online during the visa application process. Applicants will receive an IHS reference number, which is required to complete the visa application. Your BTG consultant will assist with the process.
What NHS services are covered by the IHS?
Payment of the IHS grants access to NHS services similar to those available to UK residents, including:
- Consultations with a general practitioner (GP).
- Hospital treatment.
- Emergency services.
However, certain services may still incur additional charges and it is advisable to check specific NHS services for any applicable fees.
What happens if a visa application is refused?
If a visa application is refused, the IHS fee is refunded automatically. Refunds are typically processed within 28 days of the visa decision. However, if an appeal or administrative review is requested, the refund may be delayed until a final decision is reached.
Can I get a refund on my IHS payment if I withdraw my visa application?
Yes, if you withdraw your visa application before a decision is made, you can get a refund. However, Bridging the Gap Migration strongly recommends that if you want to withdraw your visa application, you do so before your biometrics appointment. This is because it might prove difficult to withdraw your application after your biometrics appointment, making it difficult to get a refund at that stage of the visa process.
You will not get a refund if you withdraw your visa application after your visa is granted.
Also, if your visa application is successful and you do not come to the UK, you will not be eligible for a refund.
